When is home care necessary?

The decision to seek assistance from a home care agency like Quality Care Partners is often made when a family member requires help with some or all of the normal activities of daily living -- hygiene, housekeeping, cooking, shopping or even walking. It may be that more help is needed than the family can provide, or there is an increase in forgetfulness that may jeopardize a loved one’s safety.

Ideally, home care is a step that is best planned for in advance, as in the case of a home recovery from elective surgery, or when a caregiver in the family plans a vacation or simply needs relief. Quality Care Partners is also able to arrange home care on short notice.

If home care is needed, can I choose a provider?

Yes. If you or a loved one are being discharged from a hospital, nursing home, or other medical care facility and need home care services, consult with your physician or the discharge planner to set up home care services. You can ask them to refer you to Quality Care Partners.

Does home care have to be full-time?

No. Quality Care Partners offers services only on an as-needed basis, without contracts. That means we can provide assistance 24 hours a day and 7 days per week, or as little as two hours per visit. You can arrange a schedule of regular hours of home care each day, or contact Quality Care Partners when you just need a few hours away from the stresses of care giving.

Is home care really adequate for my loved one?

Yes. Home care requirements are fully evaluated on an individual basis so the most effective plan of care can be developed. Trained Quality Care Partners caregivers put the plan of care into effect. The one-to-one nature of Quality Care Partners home care meets more than just a client’s physical needs; the companionship our caregivers provide is a significant additional benefit.

Quality Care Partners is staffed 24 hours a day by a supervising Registered Nurse, providing ongoing assurance of the quality of care.

This can relieve much of the stress of a crisis, allowing families to begin arranging for regularly-scheduled service under a fully-developed plan of care.

Who monitors Quality Care Partners home care?

Quality Care Partners is licensed as a home care agency by the State of New Hampshire, Department of Health and Human Services, Health Facilities Administration, and employs a Registered Nurse to provide continuous supervision of client status and Quality Care Partners caregiver performance.

Are Quality Care Partners caregivers screened?

Yes, each caregiver is bonded and is screened in the following areas: drug testing, criminal record, driving record, and pre-employment physical.

How reliable are Quality Care Partners services?

Quality Care Partners understands that reliability is essential. Family members must be confident that caregivers will show up each and every time they are scheduled.

All of our caregivers are Quality Care Partners employees; we do not hire per diem staff.

This year our Reliability Rating is 99.9%. This means we show up when we say we will.

How will I be billed for Quality Care Partners services?

Quality Care Partners requires a deposit equal to the cost of one week’s services before care begins. The actual deposit amount depends on the number of hours of home care per week called for in the initial plan of care, or by other agreement in the case of immediate-need situations. Thereafter, the financially responsible party will be invoiced bi-weekly for services provided, with payment of any outstanding balance expected within 10 days of receipt of the invoice.

Long-Term Insurance

  • Many long-term care policies will reimburse for homecare services.
  • Consult with your long-term care insurance provider concerning reimbursement for our services.

Medicaid

Quality Care Partners is a state-approved provider of services for the New Hampshire Medicaid Home and Community Based Care, Elderly and Chronically Ill program (HCBC-ECI). If a home care client qualifies, Quality Care Partners will bill Medicaid HCBC-ECI program directly for the services they provide.

Is the cost of Quality Care Partners services tax-deductible?

Check with your accountant or financial advisor to find out if your home health care payments are tax deductible. If you pay for Quality Care Partners services directly, the unreimbursed cost may be tax deductible.

What staffing services does Quality Care Partners offer?

In addition to helping families directly with home care services, Quality Care Partners helps long-term care facilities meet their paraprofessional staffing needs. Our LNAs work in a variety of care settings including nursing homes, assisted living facilities, adult day care, and ambulatory programs.

What will home care services cost?

The hourly fee is determined by the level of care needed and the length of each visit and will range between $20-$26/hour with a two hour minimum required. Call Quality Care Partners between 8:30 and 5:00pm Monday - Friday.
